Google Docs, Google’s popular version of Microsoft Office Suite, now lets its users to create online forms.
Just sign into Google and open up Google Docs. Then, go to New > Form and the rest is history. Just follow the simple onscreen guide to create a simple online form, which you can email to your recipients or embed into your web site/blog.
After your recipients fill-in the form, you can view the summary response from your Google Docs, or you can view the full response in Google Spreadsheet format.
Even though the form is quite basic (text field, multiple choice, checkbox, drop-down list, scale (1-n) and paragraph text), it is sufficient and good enough to create many type of forms including surveys, questionnaires, etc.
